What does Formaplan F cover?
Formaplan F is for those who want to also include their families in their plan. Forma designed this package to provide some oral care as part of the package to alleviate some of the costs. Formaplan F also includes everything in Formaplan:
- The employee can arrange separate financial cover for their family.
- We are open 5 days a week and outside of working hours.

Terms and Conditions
*During your initial visit you will be given a full dental examination. You will need to be dentally fit in order to benefit from the Forma plan and any pre-existing conditions must be treated and paid for in full.*
*You are then entitled to 2 free examinations and hygiene visits per 12-month period, this is included within your plan. Children are included in Plan F.*
*Children are also assessed for orthodontic treatment when necessary.*
*The annual allowance is then redeemable towards dental treatment.*
*The annual allowance can count towards multiple treatment items but cannot exceed the annual amount.*
*All treatment costs in excess of the allowance must be paid in full.*
*Dental treatment must be agreed by the treating dentist.*
