Having braces as a teenager is no guarantee that the smile will be acceptable in your early adulthood. Retainers given after orthodontic treatment are not designed to last too more than 1-2 years, and as a teenager you may still be growing.

This patient was 22 years old with moderate upper and lower crowding, centre line shift, premolar rotation and some tooth wear of the incisors.

The 2 lower incisors were forward, which was causing the upper 2 front incisors to drift forward. The 2 upper lateral or side incisors then started to fall behind the front incisors.

All in all, an unstable situation.

We agreed Invisalign aligner treatment. It took 13 months with no teeth extractions. The patient attended every 6-8 weeks for review appointments.

Teeth can develop in many ways and sometimes an abnormal formation can cause occlusal/visual problems.

Dens in dente is an extra cusp formation usually on the incisors, which can interfere with the bite or cause tooth drift. The Dens in dente or talon cusp can be made of enamel, dentine and even nerve tissue.

It looks like a bump and sometimes you can feel it with your tongue. Most people grow up with them and think of them as normal.

They rarely are a massive dental problem.

Dental anomalies are rare and usually of a genetic nature or a disruption during tooth development, such as chicken pox or measles.

The case above shows the forward drift of the UL1 as the lower teeth bite onto the talon cusp, this can also be called an increased overjet.

The patient had a close/deep bite with a narrow arch and prominent UR1 and UL1.

Sometimes the teeth drift due to clenching, which can cause muscle forces to compress the back teeth, moving the front teeth forward, this is called arch collapse.

We advised Invisalign aligners with some minor recontouring of the UL1 incisor.

The lady wore Invisalign trays for about 9 months to expand the dental arch and bring in the upper incisors. The aligners were changed every 2 weeks. The lower teeth were also moved downwards to create space (deep bite reduction).

Patient was happy with straightened front teeth and widened arch.

Tooth whitening was done and the patient received 6 Vivera retainers that so far have lasted 10 years.

Why do you get an overjet?

Sometimes teeth can move forward from thumb sucking or pen chewing and even from teeth grinding at night.

Teeth grinding or clenching is caused by the cheek muscles that press onto the teeth, pushing the side teeth in and causing the front teeth to move forward.

If left untreated, the upper teeth can continue to push forward and upwards causing the front teeth gum to shrink/recede.

Also as the upper teeth move forward, they lose contact with the lower teeth and the lower teeth may drift upwards.

Do I need teeth out?

In most cases where the overjet is 7mm or more we recommend 2 upper teeth are removed. This happens quite frequently in children and so young or old adults should not be surprised that they too may need teeth removed.

Will extractions affect my face?

Patients with a large overjet sometimes have a long narrow face and or protruding upper or both upper and lower lips.

Extraction teeth and moving the teeth back will allow the lips to be a resting angle, and also to be flatter.

If you have very thin lips then extractions may not be advised, but with full lips, extractions will help improve the facial proportions.

Will extractions affect me later in life?

Remember most teenagers have 4 premolar teeth extracted for fixed braces, and they do not have any problems later in life.

Adult extractions are quite common with orthodontics, and much safer than interdental stripping (IPR).

The lower teeth therefore prevent the upper teeth from being moved orthodontically back.

This lady had a narrow arch and the upper teeth were sticking out. She had fixed braces as a teenager, did not wear the retainers and the teeth moved forward.

The patient was not happy with her profile but did not want a quick solution, she wanted the best solution that fitted around her teeth. She felt that her upper lip stood out and her lower lip was trapped under the upper teeth.

Her lower teeth were slightly angled forwards and the lower jaw was set back. The overjet was 12mm.
This is common in patients who have a shorter lower jaw or a longer upper jaw.

The upper teeth could have been tipped back but there would only be a slight change in the profile and overjet.
We recommended Invisalign with 2 upper premolar extractions, and the patient agreed.

2 teeth were removed at the clinic and plastic fake teeth were placed inside the aligners.

The patient collected 6 aligner packets at a time and came in every 60 days for a check up.

The aligners were changed every 10 days and taken out only for eating, brushing teeth and the odd social event.

Invisalign was used as the orthodontic treatment of choice and it took 15 months to finish the treatment.

The upper teeth were reshaped at the end although the UL1 could have been reduced more.

The patient received straightened teeth and a more relaxed profile. The lower lip no longer rested underneath the front teeth, and her lips became more relaxed and in a natural position.

Jaw surgery was not needed.

In summary what looked like a surgical case ended up as an Invisalign extraction case.

Many patients have narrow dental arches from thumb sucking or clenching their teeth. The facial muscles create forces that will compress the teeth, narrowing the dental arch and cause dental crowding.

The teeth may begin to overlap, rotate or even protrude.

Teeth out o their natural positions are more prone to damage from trauma or tooth wear.

Sometimes you may notice one front tooth is shorter than the other.

Simply by moving the teeth back to their original positions will recreate the space and solve the crowding issue.

This Invisalign case was a 28 year old student who had severe upper and lower crowding and a missing tooth.

We used Invisalign to expand the dental arch and move the teeth back to their original positions, using the missing tooth space.

It took 15 months of treatment and the patient attended every 8 weeks.

At the end we provided 6 Invisalign retainers and tooth whitening as well.

The 3 pairs of retainers are rotated every 4 months which keeps the teeth locked in their final positions.

Teeth can be repeatedly filled when broken, with bigger and bigger fillings.

Eventually the nerve may become damaged or the tooth fractures beyond repair. This can end up as months of pain and discomfort.

A gum boil can be the first sign of nerve infection.

Sometimes you can receive unsightly fillings that stand out when you smile.

So what should one do?

Firstly an x-ray could be done to see why the tooth is failing. Sometimes you can carry out root canal treatment and then crown the teeth. But there is no guarantee and the teeth could still fail underneath.

Many NHS dentists would even find the teeth above too difficult to fix, let alone remove.

If a tooth is removed then you can get tooth drift leading to an unstable occlusion.

Our recommendation is if the tooth is too expensive to repair, then replace it with an implant crown.

Placing an implant is less painful than taking a tooth out.

We drill a small hole through the gum into the bone. Bone does not contain nerves so you don’t get any pain. The bone grows around the implant.

The Titanium implant rarely fails as opposed to deep fillings or root canal treatment.

Within 3 months you can have crowns attached to the implants and your smile is restored. The crowns are attached with screws and no messy glue. The teeth are also easy to floss and to keep clean.

If you ever get a porcelain fracture, the dentist can unscrew the crown and get it repaired.

There is a new trend of Seltzer drinks, Seltzer water and even Seltzer alcoholic drinks; they all contain Carbon Dioxide.

Unfortunately dissolved carbon dioxide creates a weak acid called carbonic acid.

Acids can dissolve minerals such as enamel and dentine and are in the long term harmful to the teeth.

Enamel is 95% mineral and Dentine is 70% mineral. Both contain crystals of Hydroxyapatite and some fluoride atoms as well.

Acids react with minerals to cause the release of ions and the minerals effectively start to dissolve.

As many sip fizzy drinks from cans, the acid attack is ferquent throughout the day and the enamel slowly starts to dissolve.

Initially the teeth may looks shiny or have translucent edges, but eventually you start developing pits and cavities as above.

Most composite repairs will begin to stain or wear away, especially if one continues drinking fizzy drinks. In the case above the gentleman was 26 years old and had for many years been replacing the anterior fillings.

He wanted a better solution and we firstly asked him to drink still water and refrain from all fizzy drinks. A diet analysis was also carried out to see if he was eating anything too sweet etc.

After careful analysis we asked him to come back to discuss options.

He did not like the repeat fillings and also thought his teeth were too long or misshapen. We discussed Emax veneers as a better long term option. Veneers are known to lats over 20 years+ if looked after.

A veneer is a cover. It can be bonded onto the front tooth with no or little preparation of the tooth. In some cases we align the teeth with Invisalign firstly to get the best out of the smile, before veneering.

Veneers involve setting up a mock up of the new shape and length and if the patient us happy, we then prepare the teeth gently.

2 weeks later new porcelain Emax veneers were bonded to the teeth. You may notice some pink porcelain at the gum levels; we use this when there has been gum recession and the patient does not want extremely long teeth.

The Emax will not stain as composites do and will rarely chip or fail. The occlusion is always investigated for any irregularities beforehand.

For individual cases we use the harder and more durable Zirconia porcelain; which gives a very white smile.

Acid erosion can lead to damaged teeth and a poor smile, but modern day dentistry if minimal, can also save the teeth and restore confidence.

In the 1920s many film and theatre actors would have silicate or plastic crowns bonded to their teeth. In modern times many celebrities have porcelain veneers instead.

With veneers the smile is whiter, more cosmetic, more aesthetic and extremely confidence boosting. Although not every actor has a great smile; some dentists do not take care when providing treatment and can make their patients look ‘Goofy’.

Tooth movement of one arch can cause disruption to the teeth in the other arch, leading to crowding, tooth wear and in some cases tooth fracture.

This lady who was in her mid 40s actually saw us for a consultation 11 years ago, but never came back, due ti family reasons.

During Covid she contacted us again and asked if we could help her.

An Itero 3D scan was taken with digital images.

We noted mild upper crowding with porcelain crowns and uneven margins with moderate lower crowding and the LR2 being hidden in the smile.

We agreed on an Invisalign treatment with arch widening and proclaiming of the lower incisors.

The upper incisors were lifted up to even out the gum margins.

We did about 20 aligners over 2 treatments and the patient attended irregularly throughout the treatment. Treatment time was less than a year.

It is always amazing what results we get and the patient was quietly astonished as well.

We finished off with 3 pairs of Invisalign retainers and tooth whitening as well.

Missing teeth spaces can allow other teeth to drift sideways on downwards into the space; this ca cause occlusal and even joint problems.

Missing teeth can also create aesthetic problems, but the biggest drawback is the inability to enjoy your food. To many people, eating is the culmination of all the senses.

So when you cannot chew your food, or enjoy a hearty meal, then you can always ask us for advice. There are many options.

Replacing these missing teeth by way of bridges fixed over existing teeth is a possibility, but this can be very destructive and reduces the life expectancy of the natural teeth. Sometimes a tooth may need subsequent root canal treatment.

Dentures can be made, but they need replacing, can become loose or even break. They are usually made of acrylic or metal acrylic with metal clasps. Nowadays we can also make flexible dentures that stretch over the teeth and do not need clasps.

By way the best way of replacing teeth is using implants. Implants can support many teeth, maintain bone levels and being fixed are a natural way of replacement.

Implants are made from Titanium alloy, are bio-inert and have been used in dentistry for over 50 years.

Patient AB attended with 2 unrestored implants placed at another clinic, unfortunately the clinic had closed down. Lower Right 654 were missing.

We were using the same Nobel active implant system and we were able to help. We assessed the patient, his occlusion and his aspirations.

The patient had the implants placed 6 months ago and had lost his temporary acrylic bridge after it had fractured.

One of the implants had been placed very close to the canine tooth, which could jeopardise the nerve of the canine and also the long term integration of the implant.

We used impression posts attached to the implants and took silicone impressions.

2 weeks later a metal porcelain bridge was attached to the implants by way of integral implant screws. patient was very happy to be able to chew on both sides; now he is considering Invisalign.

Dental implants if paced with care and attention can last a lifetime.

Many patients are too embarrassed to have braces as adults. They put up with their smile (not) because the thought of fixed braces and the pain just puts them off.

They missed the opportunity as teenagers at school, because either nobody bothered to offer any help, or their smile got worse as they reached adulthood, and nobody wants fixed braces at university.

So mid 20s they wander what they can now do now that they are working and trying to move up the job ladder or getting married perhaps.

Invisalign aligner treatment was invented about 20 years ago and arrived in the UK about 18 years ago. It is a series of removable aligners, that move teeth sequentially. You can drink with them in and there is no pain.

In this case study a young woman presented with protruding teeth and multiple spaces. The protrusion is usually due to thane or thumb forces and can push out the teeth and the anterior part of the maxilla as well.

In extreme case surgery is needed to reduce the bone position/ protrusion.

After a careful assessment of the patients needs and dental occlusion, we agreed to use Invisalign aligners.

Impressions and photos were taken and aligners ordered. She came back to have the aligners fitted and the whole sequence was given to the patient. Treatment time was about 15 months, which can be reduced to 12 months now.

She was supposed to come back after 3 months for a review, but did not come back for 2 years!

Inbetween her treatment she had had 2 children and Invisalign was the perfect partner on her journey of self-discovery. She just changed aligners every 2-3 weeks and got on with her life.

We were both very happy with the results and she could not thank us enough.

I hate to say it but the improvement was amazing and she had turned into a swan.
